I get that Apple Watch knows when I’m asleep, yet it still wakes me up to say the WatchOS update isn’t done. Why Apple do you think this is okay while I’m trying to sleep? My Focus mode is set to Sleeping and the Watch tracks my biometrics to see if I’m awake. I’ve blocked many apps from sending notifications during Sleep, but these system messages still come through. It’s really frustrating!
I haven’t experienced this, but it doesn’t surprise me. The thing that annoys me is the wind down alert:
Me: (peacefully drifting off to sleep a bit earlier than usual)
Watch: Hey! Hey! Come on, it’s time to get ready for sleep!
I have all notifications silenced but it still happens. The only way I’ve found to avoid it is to set sleep focus to start an hour before I might actually feel tired.
@Hollis
I had that issue too, but you can avoid it under Settings → Focus → Sleep → Next schedule, where you can set Wind Down to 0 minutes.
I was getting notifications at 4 AM to finish my stand goal, even with Sleep focus mode on and all notifications silenced. I had been asleep for 5 hours by then. My watch certainly knew that.
I fixed it by deleting the Sleep Focus in iPhone settings and adding it back.
@Denim
That sounds really annoying. Thanks for the tip on the Sleep focus, I will try that!
Better than being yelled at to stand at 3 AM. I had to turn that feature off.

No slacking off! You need to stand!
Check your Focus settings to see if notifications from the Health app can come through during focus times. Go to Settings → Focus → Sleep → Apps. If you see anything listed (like Health), remove it.
